Hospice Admissions RN / Registered Nurse New

HealthCare Support is actively seeking a Hospice Admissions RN / Registered Nurse to fill an opening with a large nonprofit hospice company in Greater Nashville!

Daily Responsibilities for Field Nurse Practitioner / Physician Assistant:

• This is perfect for the person looking to get into hospice, looking to get away from direct patient care, or for someone who loves the patient-education aspect of nursing, paired with assessing/admitting new patients, providing compassionate care, kindness and comfort.
• Performs admissions activities in all areas of services, also performs initial physical, psychosocial, and spiritual assessment of the patient and family.
• Uses nursing processes to establish and implement a hospice plan of care in collaboration with interdisciplinary team members.
• 4x10's - 1pm-11pm OR 2:00 pm-midnight - will work Friday, Saturday OR Sunday, and two days during the week.
  
  
  

Required Qualifications for Field Nurse Practitioner / Physician Assistant:

• Clear and active RN license in Tennessee
• Valid driver’s license, own vehicle, car insurance
• Minimum 1+ years of clinical nursing experience
  
  
  

Preferred Qualifications for Field Nurse Practitioner / Physician Assistant:

• BSN
• Prior admissions experience

Pay Details:

• $70,013 - $80,245 annually
• Eligible for incentive pay up to $3,900 annually
